TAP flap for breast reconstruction

How does Flap Surgery Work?

Flap surgery involves transferring healthy, alive tissue (a “flap”) from one location of the body to the other. In breast reconstruction, excess tissue, for example from the abdomen or back, is used to reconstruct the missing breast after mastectomy.

These flaps are supplied by blood vessels which pass through muscles on their way to supply the overlying skin and fat. Methods proposed in the past took the whole muscle together to maintain blood supply. Such techniques have been progressively evolved into muscle-sparing flaps, such as the DIEP flap. The blood vessels can now be separated from the surrounding muscle using perforator dissection under magnifying glasses.

The latissimus dorsi muscle is a broad flat muscle over the back. It has functions of bringing the arm backwards and inwards towards the body. There are other muscles that have similar overlapping function; hence patients do not feel any impact on their daily activities.

In the Thoracodorsal Artery Perforator (TAP) Flap procedure, an ellipse of skin and fat is also taken from the back for breast reconstruction. However, the underlying muscle is left behind and maintains its functionality. It is indicated in certain patients such as the highly active woman who is keen to continue upper limb sports. Certain occupations such as being an air-stewardess also requires significant shoulder strength – in instances such as lifting luggage into an overhead compartment.

To increase the volume of tissue, excess fat can be transferred from thighs or abdomen and used to “plump up” the TAP flap. Depending on the breast size, this one-stage procedure may be sufficient. If not, a repeat minimally invasive fat graft session can be repeated at the patient’s convenience.

Besides complete breast reconstruction, the TAP flap can also be used as part of an implant-based reconstruction, for partial breast reconstruction after lumpectomy and to reconstruct other chest wall defects.
